make the registration process more efficient and timely, please
print and complete the new patient forms below. If possible,
please drop-off or mail us the Authorization
for Medical Records prior to your visit so that we can obtain
all the appropriate records from your prior pediatrician.
If you intend to have your teenager seen without
an adult/guardian present during the exam, please complete
the Minor Consent Form. If you or a teacher suspects
that your child may have an ADHD behavior issue and you are
wishing to consult a physician there are several forms to aid
in that assessment. There is both a Parent and a Teacher
Assessment Form that should be filled out prior to the
visit. These assessments will then be scored by the physician.
These assessments will assist
in the proper diagnosis. |
